Mrs. Inghram, RN, our Hopkins daily school nurse. Every year the Annual Update includes the Emergency Medical Authorization. This information is mandatory each year for the safety of your child(ren). All other forms and care plans are found on the Mentor Schools website under the Parent Portal Nurses forms. Please remember prescribed medication needs the form completed by the physician. ALL medication needs to be brought to the clinic by the parent with the paperwork in order for medication to be administered by trained staff.
